Acknowledgment of Danger. I am aware that participating in athletic activities—including but not limited to Private Tumbling, Open Gym, All-Star Practice, and Team Practices—is a high-risk activity that can result in serious injury.*

Scope of Potential Injury. I understand that risks include, but are not limited to: death, serious neck injury, serious spinal cord injury which may result in complete or partial paralysis, brain damage, serious injury to virtually all internal organs, serious injury to all bones, joints, ligaments, muscles, tendons, and other areas of the musculoskeletal system, serious injury or eye impairment, and serious injury to other areas of my body, general health and well-being.*

Good Health. I certify that the participant is in good health and has been cleared by a healthcare professional to engage in these high-intensity activities.*

Voluntary Assumption of Risk. I voluntarily assume all risks associated with these activities and accept sole responsibility for any injury, disability, death, illness, or loss incurred by myself or my child(ren). I am aware that participating in any type of athletic activity in a YSS Program at 2420 Comanche Blvd NE, Ste B-3 in Albuquerque, NM 87107 can be a dangerous activity involving many risks of injury.*

Release & Covenant Not to Sue. I hereby release, discharge, and promise not to sue any YSS Program, its parent organization, its affiliates, employees, volunteers, or agents from any and all claims, liabilities, or expenses arising from participation.*

Binding Effect. This release and assumption of risk is binding upon my heirs, estate, executors, and all members of my family.*

Voluntary Participation. I voluntarily agree to allow myself or my child(ren) to participate in any type of athletic activity offered in a YSS Program, such as, but not limited to, Private Tumbling Lessons, Open Gym Lessons, All-Star Practice, etc. and assume all risks associated with voluntary participation.*

Independence of Activities. I understand that activities intended to raise funds for school teams or individual cheerleaders are independent of, and not affiliated with any YSS Program.*

Administrative Fee. I acknowledge that YSS acts as the Non-Profit Organization (NPO) for Certificate of Insurance (COI) purposes only. An administrative fee of 10% will be deducted from every check received. I understand that activities in support of raising funds for the school teams or individual cheerleaders  are not affiliated to YSS or any of its affiliates.*

Distribution of Funds. Net proceeds (after the 10% fee) will be divided equally among participants. Funds will be sent directly to the designated High School for distribution based on the breakdown provided by the Lead Parent Administrator.*

Cash Prohibition. All funds raised must be submitted to “Youth Spirits Squads, Inc.” for formal distribution. Under no circumstances will cash be distributed directly to individuals.*

Allocation of Proceeds. I understand that fundraising proceeds may be applied toward program activities (such as facility rentals or team classes) but do not include individual tumbling lessons unless otherwise specified.*

I am an adult over the age of eighteen (18).*
